Woodside Health & Care Centre Registration

In order to register with the Practice you will be asked to complete a Registration Form. We will ask for medical history information and your regular medication. You will then be given a registration medical appointment with the Practice Nurse.

All patients are asked to provide photographic identification together with proof of address i.e. passport, driving license or matriculation card as well as utility bill & birth/marriage certificate.

Further documentation may be required for new citizens to the UK such as visa/permits, marriage/birth certificates, passport, pension documentation, proof of residence, student id/letter from education facility, European Health Insurance Card (EHIC).  Documentation from Job Centre Plus or the Department of Work and Pensions (DWP) or a letter from the home office.
